Treatment: Anticoagulation for pulmonary/CVS decompensation

  • The patient's clinical variables and characters are crucial in selecting the appropriate anticoagulation agent.
  • The duration of anticoagulation therapy should be decided based on the presentation and underlying illnesses.
  • Follow-up and Stop Date should be scheduled for each patient, individually, after initiating anticoagulation therapy.

This plan is for VTE prophylaxis or event associated with pulmonary or cardiovascular decompensation, or acute infectious, rheumatic, or inflammatory conditions, or immobilized patients with one or more additional VTE risk factors, or cancer patient on thalidomide or lenalidomide.
